Home and Community Care Program for Young People (HACC-PYP)
This program is designed for vulnerable individuals from birth to 65 years old who face difficulties in independent living, community integration, and maintaining their lifestyle. These difficulties can arise from both physical issues (disabilities, limited mobility, challenging housing, and social conditions and psychological issues (clinical depression, autism, mental health issues, psychological disorders). The program provides one-time, periodic, or ongoing support to clients.
What can we do within this program for our clients?
- Assist in understanding government programs that can offer direct support
- Help with adapting to the community (both Russian-speaking and local)
- Provide information about community events
- Assist with filling out applications for Centrelink, contacting NDIS, and other programs
